Thank you for signing up to take the CDR and NCHEC approved RD, DTR CPE, CHES^®/MCHES^® CECH self-study self-paced and online course on the Advocating for Health and building on the progress of the Affordable Care Act.

Feel free to send email to clarify course requirements and tell me about problems you may have while doing the course. Please remember that you will need to take that 20-question test and pass with a score of 70% or greater (39 - 55 points). Also, NCHEC requires each participant to complete an evaluation (included at the end of the test form). Both must be completed to get the 10.0 CPE or CECHs for RD, DTR and CHES^®/MCHES^® (includes 8.0 MCHES^® advanced hours).

20-question TF, multiple choice and short answer post-course assessment. Be sure to download and save form in directory or folder on your computer. Next, start Adobe Reader DC and use it to find and open from the saved location. Your score, when using Adobe Reader DC, will be calculated on the 2nd to last page and you can go back and changes answers to change the score. Save after completion and attach to an email to this Health Education Partners continuing education staff with this email address - ce@healthedpartners.org.
